Relationship Checking Upgrade Form

Thanks for your interest in upgrading your checking account to one that rewards you for the way you bank!

This upgrade form is provided for your convenience in switching to one of the new Relationship Checking accounts. If you need assistance in selecting an account, please call us at (515) 432-6200 (Monday-Friday, 8 a.m.- 4:30 p.m.) or stop by one of our branches.

In the form, enter either your current checking account number or debit card number of the account that you wish to upgrade. Then review the “Truth is Savings Disclosure” and mark “Accept Disclosure”.

Your account will be converted within two business days; and you will receive a summary of your new accounts by email. Then, watch for the rewards on your next full statement cycle when you meet the qualifications.

This form allows you to upgrade up to two checking accounts. If you have more than two accounts, simply complete another one of these upgrade forms after your first submission is complete. Repeat, as necessary, until all of your checking accounts have been upgraded. If you are the primary account holder on a minor’s account, be sure to complete an upgrade on that account as well.

If you wish to change your account again, no problem! Simply submit another form or call and talk to a Customer Service Representative.
